Anyone can refer a patient to hospice. Admission will include communication with the physician to determine medical eligibility. Qualifying patients are those:
With a life expectancy of 6 months or less if the illness runs its normal course;
Who desire palliative treatment;
Who desires a hospice team to administer care at the place of residence
During admission, our nurse will visit you and/or your family to discuss hospice services, assess your immediate needs and collaborate to formulate a plan of care that you approve.
