What is Hospice?
Hospice treats you, not the disease. The focus is on care, not cure. A team of hospice professionals and volunteers addresses your and your family’s medical, social, emotional, and spiritual needs. Hospice services are extended to your family, with your authorization. You and your family are included in the decision making process. Hospice will help you and your family makes choices about end-of-life issues and enables you to have greater control over these choices. Bereavement counseling will be provided to your family for up thirteen months.
Hospice offers palliative care, rather than curative treatment. Hospice will provide care and comfort when cure is no longer an option. Through ever advancing technology, pain and symptoms control will enable you to live as fully and comfortably as possible. Hospice promotes quality of life.

Criteria for Admission
Anyone can refer a patient to hospice. Admission will include communication with the physician to determine medical eligibility. Qualifying patents are those:
• With a life expectancy of 6 months or less if the illness runs its normal course;
• Who desire palliative treatment;
• Who desire a hospice team to administer care at place of residence
During admission, our nurse will visit you and/or your family to discuss hospice services, assess your immediate needs and collaborate to formulate a plan of care that you approve.
Service Hours
The Agency is open Monday through Friday from 9AM to 5:30 PM. These are our normal business hours. Times after this would be considered After Hours. The Agency has coverage 24 hours 7 days a week.
A registered nurse (RN) is available 24 hours a day, 7 days a week to assist you with concerns with regular office hours as well as being available on-call when the office is closed. We will talk with you by phone to determine your needs. When appropriate, a nurse will come to your place of residence. We are available after regular office hours for urgent conditions.
